To submit a Data Privacy Data Subject Request (DSR) on behalf of a customer who has contacted your council, please complete the form below. Please submit only one request per person to create separate cases for tracking purposes.

Provide the following information to help us locate the customer’s records in GSUSA systems. If the customer has an account with Girl Scouts, please provide the information on their account. If you are not sure, please provide the data you believe we are most likely to have on record. We will use the information we request below only for the purpose of verifying the customer’s identity and/or locating their records in GSUSA systems. We will not process the request if we are unable to reasonably verify the customer’s identity.
